アトピー性皮膚炎の病態解明と新規治療法の開発

はじめに

アトピー性皮膚炎の病態解明と新しい治療戦略の開発を目指して研究を行っています。特に、アレルギー性炎症と皮膚バリア機能の関連性に着目し、両者を同時に制御し得る治療法の開発を目指した研究を続けて来ました。更に、アレルギー性炎症や皮膚バリア機能研究を通じて、皮膚恒常性維持とその破綻メカニズムの解明に寄与できればと考えております。以下に、現時点で公表可能な研究課題と成果を示させて頂きます。

研究課題と主な成果

Th2サイトカインが皮膚バリア機能異常に及ぼす影響

Th2サイトカインが透過性バリア機能や角層固着機能に対して負の影響を及ぼすことを見出しました。

Th2サイトカインがかゆみに及ぼす影響

Th2サイトカインが神経反撥因子のsemaphorin 3Aの発現低下を介して表皮内への神経伸張を促進することを見出しました。

アトピー性皮膚炎の病態におけるリポセンサー

リポセンサーは、皮膚のバリア機能と炎症の両者と関連する因子です。アトピー性皮膚炎マウスモデルにおいて、リポセンサーのリガンドの外用が治療効果を示しました。特に、PPARαの低下が、アレルギー性炎症と皮膚バリア機能異常の両者に同時に関与し、両者間の悪循環にも関与していると考えています。

アトピー性皮膚炎の病態におけるバリア関連分子異常の意義

表皮角化細胞の重層培養系を用いた研究により、フィラグリンやロリクリンの発現をノックダウンした表皮角化細胞では、炎症性サイトカインの発現が亢進することを見出しました。皮膚バリア機能関連分子の低下は、表皮角化細胞における炎症に関する機能変化も生じさせる可能性を示唆しています。

角層pH上昇のアトピー性皮膚炎の病態における意義

角層pHは、通常、弱酸性に維持されておりますが、様々なバリア破壊刺激で上昇します。角層pH上昇後に生じ得る反応には、PAR2依存性、非依存性の様々な経路が有り、アトピー性皮膚炎の病態に深く関与している可能性があります。実際、アトピー性皮膚炎でも角層pHの上昇が観察されております。我々は、フィラグリン遺伝子異常を有するFlaky tailマウスにおける、アトピー性皮膚炎様皮膚炎の生じ易さに、角層pH上昇後の回復能の低下が関与している可能性を見出しました。アトピー性皮膚炎の発症と悪化において、角層pH回復能の低下が、皮膚バリア機能関連の遺伝的要因と環境要因とを繋ぐ重要な因子のひとつではないかと考えています。更に、ハプテン誘導性のマウスのアトピー性皮膚炎様皮膚炎モデルを用いた検討により、アトピー性皮膚炎の病態では、PAR2依存性・非依存性の両経路が重要である可能性を見出しました。
