Hospitalization Procedures
Items to be brought by all hospitalized patients
- Medical examination ticket
- Health insurance card
- Hospitalization Pledge(Continued Hospitalization Pledge)
- Personal Stamp
- Hospitalization receipt form
- Application for the loan of a hospital gown
- Sympathy Information Request Form
Items to be brought only if you qualify
- Certificate of eligibility for children's medical expenses (for high school students and younger)
- Certificate of Maximum Amount Application (for persons under 70 years old) *Please complete the procedure before admission
- Senior Citizen's Beneficiary Certificate (for those 70 to 74 years old)
- Medical insurance card for the elderly (for those 75 years old and over)
- Medical care certificate for the disabled (certificate of receipt of medical expenses for the severely mentally and physically disabled)
- Nursing care insurance card
- Certificate of Reduction (Reduction in the cost of meals; households exempt from municipal taxation)
- Certificate of discharge from the hospital (if the patient has been hospitalized in another hospital for the same injury or illness within the past three months)
- Primary Palliative Care Screening Sheet (distributed to eligible patients only)
- Copayment Limit Management Form (for eligible applicants only)
Please report to the admission desk if you are
- If you are receiving public assistance for medical fees, such a s for general medical assistance, preventative pediatric medical care, r ehabilitative medical care, or pediatric chronic illness.
- If you are being admitted due to a workplace injury (accident, civil work injury), traffic accident, or injury caused by a third party.
- If you cannot produce an insurance card and might pay hospital fees out-of-pocket.
- If your address, phone number, or insurance details change whil e you are admitted.
For long term hospitalization, we will check your insurance card every two months.
What to bring to the hospital
Please keep your luggage within the limits of what can be stored in your locker.
- The "Hospitalization Information" booklet
- Medications (including over-the-counter medications and nutritional supplements) that you are currently taking
- Medication handbook or medication manual showing the medication you have been prescribed.
- Washing and bathing utensils (soap, shampoo, toothpaste, electric razor, comb, etc.)
- Underwear (flat-rate diapers are available for those who use diapers)
- Sleepwear (available at the hospital for 84 yen per day)
- Chopsticks, spoons, teacups, etc.
- Denture container if you are using dentures
- Towels and bath towels
- Tissue paper
- Indoor footwear (non-slip-type shoes)
- Earphones for TV (available at the hospital store)
Things to keep in mind
Manicures (hands and feet) and gel nails and false nails should be removed before you are admitted to the hospital.
